All exams provided at Advanced Imaging Center, LLC require an order from a physician or health care provider. There are some exceptions for written orders, such as for screening mammograms and vascular screening US and 3D/4D US exams; patients do not need written order, however a Physicians name to send the results of the exam to is required. Advanced Imaging Center will try to accommodate walk-in appointments for most exams.

FOR CLINICIANS
Referring Doctors’ Offices may contact our office to receive exam order pads and patient information brochures.
At the time of scheduling, the following information should be provided:
- Patient name, address, and phone number
- Date of birth and social security number
- An appropriate reason for exam
- Previous imaging information
- Type of medical insurance, and any pre-authorization information
- Whether it is a workers compensation, automobile accident, or third party liability case
FOR PATIENTS
Here are some general instructions and information to consider before your exam, for more information on a particular exam, please visit our Procedures page
- On occasion, the doctor will hand the patient a written order and ask the patient to schedule the appointment themselves. In such cases, the patient will need to contact our office to set up the appointment at (309) 743-0445
- Many of our exams require special instructions and preparations. Most likely, your doctor or health care provider will tell you about this during your visit. If you have any questions about the instructions for your specific test, please call our office at (309) 743-0445
- When you arrive at Advanced Imaging Center, you will be asked to provide a few things:
- a photo id
- a current insurance card (if you have one)
- a copy of the order from your doctor (the doctor may have already faxed this to our office)
- a copy of any previous imaging (if you have any)
- Please let us know of any allergies you might have to contrast (xray dye, iodine)
- Please let us know if you are pregnant or could possibly be pregnant
- You must bring a copy of the Power of Attorney (POA) if there is a POA in place
Patients are ultimately responsible for their bills. Before your exam please consider the following:
- Patients are responsible for finding out if their health insurance plan requires any kind of authorization or notification prior to exams. Insurance companies may call this process any of the following:
- pre-authorization, pre-certification, prior notification, pre-notification, RQI*, and/or pre-determination
- Whether your health insurance coverage has any limitations, and preparing for any potential financial responsibilities
